NHIF Revenue Leaders: St. George, Sofiamed, and Pirogov

The leader in the 2025 NHIF revenue rankings is St. George University Hospital – Plovdiv with nearly 193 million BGN, followed by Sofiamed and Pirogov. This is according to data provided by the Fund. Private hospitals are taking an increasingly prominent place among the leading medical institutions—Sofiamed, Pulmed, Acibadem City Clinic Tokuda, and Heart and Brain demonstrate that the private sector is now a structurally determining factor in hospital care financed by public funds.

The ten hospitals with the highest NHIF revenues clearly outline the trend toward the concentration of public resources in large multidisciplinary and university medical facilities. What they have in common is a high volume of activity, a wide range of clinical pathways, and their role as reference centers for complex and highly specialized cases.

The strong presence of university and state structures such as Pirogov, the Military Medical Academy, Alexandrovska, and St. Marina underscores the importance of emergency care, training, and highly specialized medicine as primary drivers of NHIF expenditures.

The ranking also clearly outlines a geographical concentration—Sofia, Plovdiv, and Varna dominate, while regional hospitals remain with far more limited financial resources. This brings to the forefront questions regarding the balance within the system, cost efficiency, and the sustainable development of the hospital network.
